Contract Duration: 14 weeks
Provide diagnostic evaluations and therapeutic interventions for residents with communication, cognitive, and swallowing disorders.
Develop and implement individualized treatment plans based on patient needs.
Maintain accurate, timely documentation in accordance with regulatory and facility standards.
Collaborate with interdisciplinary team members to ensure coordinated patient care.
Educate patients, families, and caregivers on treatment goals and strategies for communication improvement.
Participate in care plan meetings and interdisciplinary discussions as required.
Monitor progress and adjust treatment plans based on resident response and clinical findings.
Ensure compliance with federal, state, and professional practice guidelines.
Provide therapy services across two assigned facilities as part of a split role.
