Job Description
We are seeking a compassionate and experienced Case Manager Level II to join our team and support children and their sponsors through post-release services. This role is essential in providing trauma-informed care, conducting thorough assessments, and ensuring children and their families receive the resources and guidance they need to thrive.
As a Case Manager Level II, you will work closely with diverse populations, including immigrant families, and collaborate with community resources to meet their unique needs. Your ability to assess circumstances, communicate effectively, and build trust with clients will be key to success in this role.
Responsibilities
Case Management: Manage a caseload of post-release services cases, providing ongoing support through assessments, planning, and psychoeducation.
Assessments: Conduct home studies and comprehensive assessments to ensure a child's safety, well-being, and proper placement.
Collaboration: Work with care providers, community resources, and sponsors to verify and address needs, risks, and additional support opportunities.
Communication: Maintain regular contact through telephone, virtual, and in-person home visits with children and their sponsors.
Trauma-Informed Care: Utilize trauma-focused approaches and brief therapeutic techniques in interactions with clients.
Documentation: Ensure accurate and timely documentation while safeguarding confidential and sensitive information.
Reporting: Identify and report any concerns related to children, sponsors, or significant incidents as a mandated reporter.
Professional Development: Participate in training, meetings, and quality improvement initiatives.
Travel: Conduct in-person home study and post-release visits, which may occasionally require evening or weekend availability.
About You
You're an empathetic, detail-oriented professional with experience in trauma-informed care and case management. You excel at building trust with people from all backgrounds, managing sensitive situations with care, and creating effective solutions under pressure. You bring:
A Bachelor's degree in Social Work, Education, Psychology, Sociology, or a related field.
At least 1 year of case management experience (including writing professional assessment reports).
Knowledge of family preservation, kinship care, and trauma-informed practices.
Bilingual fluency in English and Spanish (written and verbal).
Proficiency in Microsoft Office and data entry systems.
A valid driver's license and availability for national travel as needed.

Work Schedule
This full-time, non-exempt position requires 40 hours per week, typically across five days. Flexibility may be needed to meet deadlines and client needs.
